Practice & Case Studies

The following case studies document observations from everyday care practice. Each is structured using the PMEE template — not to reach definitive conclusions, but to develop a habit of careful observation and thoughtful reflection.

All case studies are anonymised. Names, identifying details and specific settings have been changed or omitted. These observations are practice-based and developmental — they are not clinical case reports or validated research findings.

About the PMEE case study template

Each case study follows a consistent structure: Context, Observation, Hypothesis, Intervention, Resident Voice, Outcome, Evidence, Reflection, Limitations and Next Step. This structure is designed to support learning and reflection — not to produce clinical evidence.

Context

The setting, the person, and the circumstances of the observation.

Observation

What was noticed — behaviour, expression, response, engagement.

Hypothesis

A tentative interpretation of what the observation might mean.

Intervention

What was tried in response to the observation.

Resident voice

Any verbal or non-verbal communication from the person themselves.

Outcome

What happened as a result of the intervention.

Evidence

What was observed that supports or challenges the hypothesis.

Reflection

What was learned from this experience.

Limitations

What remains uncertain, unknown or untested.

Next step

What might be tried or explored next.
